MedsCheck


Managing multiple medications or diabetes can be confusing which is why Conway’s is a strong advocate for MedsCheck, a provincial government program.

MedsCheck is a program that allows you to schedule a 20 to 30 minute one-to-one meeting with your Conway’s pharmacist to ensure you are safely using your medication. From time to time, new medications can be added to your daily routine, and an annual check is an easy way to ensure that your medication is working together for your health. MedsCheck is for Ontarians taking a minimum of three medications for chronic conditions.

MedsCheck is free for Ontarians. Just bring your OHIP card to Conway’s and we can schedule you an appointment. This review is for prescription, over-the-counter and alternative medications.

If you are not able to attend to the pharmacy, we can come to you! Just give us a call and we will provide your MedsCheck at home. We will also provide a medicine cabinet clean-up and be able to remove any medication that is unused, and properly dispose of it at the pharmacy.

MedsCheck for Diabetes

MedsCheck for Diabetes is for anyone who is diagnosed with type 1 or type 2 diabetes. MedsCheck for individuals with diabetes is available even if the individual is only taking one medication. An individual who is newly diagnosed with diabetes and taking less than three medications can benefit from the one-on-one consultation with a Conway’s pharmacist to assist with overall disease management.

It is an opportunity for our pharmacists to engage individuals with diabetes in a focused medication review including advice, training on devices, monitoring and diabetes education.

MedsCheck for Residents of Long Term Care Homes

For individuals living in a long-term care home, MedsCheck is available on a quarterly basis, and includes an annual in-depth medication analysis.

The quarterly reviews include medication selection, dosage, hours and route of administration, duration of therapy, treatments, allergies and drug interactions. They will identify any possible drug related problems that may require a more in-depth therapy analysis (annual review) and follow-up in collaboration with the individual, their caregiver and their health care team.
